FHTT Opens Second Location at McConnell Medical Center
December 8, 2025Cornwall Ontario – After expanding their unique health service model into the McConnell Medical Centre earlier this year, From Here To There (FHTT) Medical Concierge has been able to help hundreds of new patients looking for primary care and support.
“Opening this second clinic space has allowed us to hire more nursing staff and offer twice as many appointments per day,” explained Mr. Leo Liao, founder and manager of FHTT. “This means Cornwall patients can now receive care even faster and usually the same day.”
FHTT Medical Concierge aims to bridge a critical health care gap felt by residents without a primary care provider. Their main goal is to help patients navigate the health care system so they can get the care they need without having to visit the emergency room. Services include arranging telehealth family physician appointments and referrals, remote diagnostic testing including basic lab tests, simple wound care, expanded health care planning, medical form administration, and medical exams for commercial licenses.
“I have been a nurse with FHTT since the first location opened in 2019, and I continue to be amazed at how many new patients we see every week,” said Ms. Jasmin Forget, RPN.
New patients can book their first appointment online by selecting one of the Cornwall locations or the virtual “Visit From Home” option. Each concierge appointment is 15 minutes and costs a $22.60 administrative fee. All physician and diagnostic services offered during the appointment are covered by OHIP. New Canadians or visitors without an OHIP card will need to pay for the full medical service, if needed.
“Being in the McConnell Medical Centre has been a game changer because we can quickly go down the hall to get x-rays and lab results when they are ready,” explained Ms. Forget. “This new location really simplifies and speeds up the process for everyone involved, especially our patients.”
FHTT Medical Concierge is now accepting new patients and open Monday to Friday, 9:00am – 5:00pm. Their new location is in Unit 128 of the McConnell Medical Centre located at 820 McConnell Avenue, and the original location is part of the Walmart Pharmacy Health Centre, located at 420 Ninth Street West.
